Heartland Cognitive Care Collective is a non-profit 501(c)3 dedicated to improving early detection, prevention and intervention outcomes for individuals and their communities facing Alzheimer’s Disease and Related Dementias (ADRD).
Heartland Cognitive Care Collective was formed in 2025 out of a small group of dedicated professionals collaborating around the BOLD initiative.

We are proud members of a public private partnership that is focused on practical improvements at the community level. Board members include individuals the University of Iowa, University of Northern Iowa, Broadlawns Medical Center, My Community Hub, My City My Health, Iowa Health and Human Services and the Alzheimer's Association of Iowa.
We all appreciate a summit loaded with information but, often, we walk away with little connection back. The Heartland Cognitive Care Collective is different - we aim to meet regularly after summits to gather updates from participants, monitor data for improvements and opportunities as well as transparently report on the Collective's work.
